Salivary cortisol has always been a useful tool for measuring and monitoring stress in individuals particularly in a research setting. This tool is now very much moving into a diagnostic setting with the use of salivary and hair cortisols as a part of routine psychiatric investigation. Methodological validation is important to ensure that the methods employed in research trials are validated to a standard expected in pathology laboratories and recommended by ISO15189 and local guidelines. This project will utilise salivary cortisol samples collected for two ongoing research trials to validate the Salimetrics Salivary Cortisol assay and provide researchers with quality assurance around their results.

The aim of the study is to develop culturally appropriate mental health assessment tools for older adults (e.g. aged 45 and over) living in the Torres Strait. Data from yarning circles with Torres Strait community members and health care staff will be used to identify how mental health disorders are expressed in the Torres Strait. A Delphi process will then be used to identify appropriate tools for assessing these domains of mental ill health for use in older adults in the Torres Strait. These tools will then be piloted to determine acceptability and validated for use in the Torres Strait.

The proposed project will investigate the best way to assess carer burden in the Torres Strait. Research suggests that current approaches to assessing carer burden may not accurately capture the Indigenous perspective of caregiver wellbeing. Yarning circles will be used to investigate carer experience in the Torres Strait. Yarning circles will focus on key issues such as the experience of burden, barriers to accessing support and providing care, and the health service response to carer issues. Findings will guide the development of a culturally appropriate framework for the assessment of carer burden, and provision of supports and tailored interventions.

5000 Australians with diabetes have a leg amputations each year. This project investigates a novel drug therapy approach to reducing the need for leg amputations within an established rodent model. The aim of the study is to assess if administration of a direct activator of adenosine-5'-monophosphate (AMP)-activated protein kinase (AMPK) will promote healing within a rat model of diabetes and limb ischaemia-associated foot ulceration.

Interlukin-6 is an established inflammatory marker that has been shown to be increased in individuals with diabetic foot ulcers (DFU). Furthermore, DFU has been referred to as an inflammatory state. Therefore, a reduction in inflammatory markers in this high-risk population is likely to correlate with a corresponding improvement of diabetic foot ulcer healing. We aim to study in this pilot clinical trial the effect of Vildagliptin on regulation of IL-6 and its effects on diabetic foot ulcers healing.

This project aims to investigate the feasibility of processing the PPE waste materials using a Microwave Assisted Waste processing technology developed at James Cook University. This project will develop a North Queensland Liver Cancer Library. Liver tumours and non-tumour tissues will be resected from patients and fixed and frozen. The tissues will also be dissociated and cultured in three dimensions to make tumour organoids that will be used in future in vivo experiments in mice, drug trials and biochemical assays.
Metformin for treating peripheral artery disease-related walking impairment (MERIT) (Old ID 24975)

Blockage of the lower limb arteries (peripheral artery disease; PAD) leads to severe leg pain, walking impairment, and a substantial risk of leg amputation and death. Approximately 200 million people worldwide and approximately 1 million Australians have blocked leg arteries. This problem has recognised treatment deficiencies in comparison to other common diseases, including the absence of effective medications to increase blood supply to the legs, reduce leg pain, improve walking ability and reduce the risk of major amputation. Multiple lines of evidence suggest that metformin, a cheap and safe medication, promotes formation of new vessels, improves microcirculation and muscle function, and limits pain. In preliminary studies, metformin significantly increased blood supply to the limb of a pre-clinical model of blocked leg arteries. We have also associated metformin prescription with a 4-fold reduction in the rate of major lower limb amputation in patients with blocked arteries. This placebo-controlled, randomised clinical trial will examine the efficacy of metformin in improving walking ability in patients with blocked leg arteries over 6 months. Positive findings from this trial will identify a new treatment for a problem that affects 10-20% of adults aged over 50 years.
