Abstract

The “revolving door” phenomenon of early rehospitalizations of patients with advanced chronic liver disease arises from the interaction of numerous, often multilayered and challenging, factors. Delivering integrated and effective secondary preventive care has the potential to close the revolving-door phenomenon of frequent readmissions and reduce pressure on inpatient services. To address this, we launched the Care Coordination of Liver Disease (CCoLD) pilot, a novel nurse-led cirrhosis clinic in western Sydney.
